Voluntary Participation

1. I, ________________________________________, acknowledge that I have voluntarily applied to participate in all activities associated with the On The Rocks 4 Wheelers.

Assumption of Risk

2. I am aware that this activity could be hazardous. I am voluntarily participating in this activity with the knowledge of the possible danger involved. I hereby agree to accept any and all possible risks of damage to my vehicle, self and occupants riding with me as a possible result of this activity. I will not hold the On The Rocks 4 Wheelers club, its officers or its members liable. Initial _________.

Release

3. As a consideration for being permitted by the On The Rocks 4 Wheelers club to participate in this activity, I hereby agree that I, my assignees, heirs, distributes, guardians, and legal representatives will not make a claim against, sue, ore attach the property of On the Rocks 4 Wheelers, its officers or its members for injury or damage resulting from negligence or other acts, howsoever caused, from all actions, claims, or demands that I, my assignees, heirs, distributes, guardians, and legal representatives now have or may hereafter have of injury or damage resulting from my participation in the above described activity. Knowing and Voluntary Execution

4. I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between myself and the On the Rocks 4 Wheelers club and its officers and members. I am signing this of my own free will.
